Job Description: |
Job Descriptions/Responsibilities:
- Conduct quality anthropometric measurements for referred beneficiaries at the Program, ensuring efficient workflow.
- Compile, organize, and maintain accurate records of beneficiaries referred to FC using the program\\\'s standard recording format.
- Address and resolve beneficiary complaints related to records and admission criteria.
- Counsel all patients for follow-ups through Community Outreach Officers.
- Collaborate with outreach teams to establish and maintain a community-level referral system.
- Engage HCPs/LHWs in referring malnourished children, pregnant, and lactating women to FC.
- Conduct awareness sessions on mother and child health at FC.
- Assist in developing fact sheets and training materials as required for MOH health staff.
- Provide technical support to MOH staff in organizing meetings and addressing project-related queries.
- Collaborate with health facility staff for implementing nutrition services effectively.
- Identify case studies and write success stories.
- Ensure proper handling and calibration of anthropometric equipment; report any issues to the OTP/SFP Supervisor.
- Assist in preparing reports on admission, discharge, defaulters, graduates, and absentees throughout the project cycle.
- Attend scheduled staff meetings at FC as directed by the Coordinator.
- Actively participate in workgroups at FC to support project goals.
- Build strong relationships with health center staff and community leaders.
- Establish networks with other stakeholders in the working area.
- Foster community understanding and support for the program.
